Malaria is a mosquito-borne disease that affects more than 200 million people each year. That means on a global average, malaria sickens 1 out of every 20 people each year, a total that is roughly comparable to the population of Brazil, 202 million. There are more than 100 species of the malaria parasite, but the most common and deadly type is known as Plasmodium Falciparum. Once the parasite enters the bloodstream through a bite by a certain type of mosquito called the Anopheles mosquito, the parasite enters the liver where it then multiplies over 10,000 times through asexual reproduction. Approximately 2-3 weeks after entering the liver, the parasite bursts back into the bloodstream where it starts to infect red blood cells. Symptoms of malaria include diarrhea, profuse sweating, nausea, headaches, vomiting, and high fever. Without the correct drugs and treatment, malaria infection can develop into anemia, hypoglycemia or cerebral malaria, which will eventually block the flow of blood in the capillaries carrying blood to the brain. This can cause coma, life-long learning disabilities, and even death.

Different factors are debated when it comes to what can influence the spread of malaria, including the movement of the human population, as well as the state of the economy and the condition of public health. Although debates continue on about these variables, one conclusive factor that all scientists agree upon is that malaria is climate-sensitive. A certain balance of temperature, rainfall, and humidity can create ideal conditions for mosquitoes to breed and transmit malaria parasites. According to an analysis of malaria over the past two decades, scientists have come to the conclusion where the millions of people living in higher altitudes of tropical countries are at a higher risk of contracting malaria. In 2014, research teams from the University of Michigan and schools in London searched through the records of Ethiopia and Colombia and found all the relevant information about the research conducted by Menno Bouma, a professor at the London School of Hygiene and Tropical Medicine. Starting in 1994, Bouma and his colleagues were analyzing malaria’s occurrence rate in two mountainous regions in Colombia and Ethiopia. The data they collected proved that as time went on and the temperature rose, the amount of Malaria incidences significantly increased in populations living 1200 meters above sea level, Bouma stated: “We have identified that Malaria does indeed move up and down in altitude and that the movement depends on temperature”. The research teams from the University of Michigan and schools in London kept searched through regional records in Ethiopia and Colombia, and found that Dr. Bouma also said: “The implications are that if this is true, and that a global warming is causing an increase in temperatures, then malaria will increase at higher altitudes where many people live.”

As time goes on, these towns and villages at high altitudes are more and more of a risk. These areas are relatively populated, with medical facilities that not only lack in quantity but also lack in quality. The reason the areas with a higher altitude are more populated is a higher amount of rainfall, which although may attract mosquitos, is luxury because of the source of fresh water. The weather was also significantly cooler as you got higher up the mountain, with a more comfortable temperature and a smaller amount of dangerous animals. These two reasons made it a privilege for people over generations to live at a high altitude. But with the temperature increasing at the rate it is, the malaria infection rate will be similar to the rate at sea level. Also, these populations have lived in the mountains for many generations, and will not have developed any natural immunity from the disease. Making the disease all the more severe when it spreads to higher altitudes.

As the amount of human malaria infections is increasing, people should be taking extra precautions to prevent catching malaria. One of the most important ways to avoid catching malaria is to not get a mosquito bite in the first place. People should always be using mosquito repellent that contains pyrethroids in all living and sleeping areas, especially in the evening and night hours. Also, if you can bear wearing long-sleeved shirts and long pants, do it. The less skin showing decreases the risk of getting bitten. You can also apply permethrin, a synthetic insecticide used against disease-carrying insects, to your clothes before putting them on. People should also avoid sleeping outside especially in the vicinity of any body of water and make sure there are no holes or any space, which mosquitos possibly can get into your home whether it be a tent or a house. This can significantly increase the success of your malaria prevention effort.

Public health officials strongly recommend people should avoid traveling to areas where malaria is common, especially for young children and pregnant women. Diabetes is a long-term health condition. As more and more diabetes cases are occurring in recent years, more and more questions are being asked about this disease. According to the latest MMWR report (Morbidity and Mortality report), from 1995 to 2010, 100% increase of diagnosed diabetes cases reported in 18 states of the United States, with a 50% increase in the other states at the same period of time. Based on a 2014 CDC report, a massive 9.3% of the American population have diabetes. In 2012, of the total of 29.1 million, 8.1 million were un-diagnosed.

It is not only the United States that is having a drastic increase in diabetes cases. In the year 2000, India had an estimated 31,705,000 diabetic population which is estimated to increase over 100% to 79,441,000 by the year 2030, according to an official WHO estimate. In the year 2015 in Australia, around 1.7 million people had diabetes, this doesn’t include un-diagnosed diabetes (estimated 500,000). That was approximately 11.04 % of the population in 2015.

People who have diabetes, but don’t realize they have it yet are at the highest risk of health problems, this is because not treating the diabetes can lead to very bad consequences. Your cholesterol level rises, your vision fades, your kidneys may start to fail, in some cases we have seen it lead to patients losing lower-limbs. This happens when the disease causes damage to the nerves between brain and lower limbs. An accident or a wrong step can lead to an ulcer in your foot, and because of numbness and poor circulation, the healing process becomes very slow. The end result is an infection that spreads to the bone, which then gives you no choice but to amputate the foot. Due to these above serious consequences, doctors and epidemiologists are striving to not only find the cure to this terrible disease, but also try to figure out the main reason behind the significant and dramatic increase in the number of diabetes diagnoses. One of the main theories initially brought forth was that diabetes had a link with the genes in human DNA. So is the reason you have diabetes or know someone that has diabetes possibly because of genes? The answer is yes.

Diabetes is a condition where there is too much sugar (glucose) in your bloodstream. This occurs when your pancreases ceases to create enough insulin, a hormone that acts as the transporting agent of glucose into the bloodstream. When there is a lack of insulin, there is a shortage of glucose is entering your body’s cells. It is also possible that there is in fact a sufficient amount of insulin being produced by your pancreas, but is not working properly. There are two types of diabetes divided into type 1 (juvenile onset) and type 2 (adult onset), type 1 meaning there is no insulin to transport glucose in the first place. This type is usually developed when you are a child. Type 2 diabetes is when there is not enough insulin to carry the glucose around the body or there is enough insulin but it is not working properly.

Are you at risk?

If one of your loved ones has diabetes, you might be worried about developing the disease yourself, or even passing it onto your children. Many researchers and scientists have been studying diabetes hereditary factors, and while a lot has been learned about what genetic factors may increase the chances of someone getting diabetes, there are still a lot of unanswered questions. For more than 20 years researchers in the epidemiology and genetic section in a Boston hospital called Joslin have been analyzing past diagnoses and cases of diabetes. According to their research, there are certain factors that can fluctuate the risk of a human: the person that got diagnosed with diabetes most likely inherited a predisposition to the disease. Secondly, it is possible that something in the environment has triggered the disease.

Are you at risk of getting type 1 diabetes?

At the Joslin Diabetes Center, a non-profit institution affiliated with Harvard Medical School, researchers and lecturers have conducted research about the relationship between genes and the disease for over 20 years. Researchers including section head Andrzej S. Krolewski, M.D., Ph.D., and Senior Investigator James H. Warram, M.D., Sc.D., have been continuing the research that began in 1946 by Elliott P. Joslin, M.D. Over the years, these two researchers and their colleagues have studied many different generations of families to determine ways to predict who is at risk of diabetes. They came to the conclusion that one’s risk of developing type 1 diabetes can go from a 1% chance to a 10% chance or higher, depending on whether or not a family member has developed diabetes and when they did. For example, if one child in the family has type 1 diabetes, then their sibling have a 10% chance of developing it by the age of 50. Another conclusion that researchers have come to is that the risk for a child of a parent with type 1 diabetes is significantly lower if it is the mother rather than the father who has developed diabetes. If your father has developed type 1 diabetes, then his child will have a 10% chance of also developing the disease. However, if that same child’s mother was the one that developed the disease, the chances of developing diabetes drops down to 4%, and if the mother develops it after she is 25, the chance drops down to 1%.

About type 2 diabetes

For type 2 diabetes, if patients know a relative that has developed it, it is more likely that it “runs in the family”, compared to type 1. This doesn’t always mean it has to do with genes, sometimes it is due to environmental factors. For example, obesity and an unhealthy lifestyle could cause a higher risk for all members of the family. In most cases, the risk of someone developing diabetes when his or her sibling has it is around the same as that of someone in the general population. However, there are some exceptions to this case. If the patient has a relatively healthy diet and exercises regularly, then the siblings risk doubles in comparison to the risk of someone from the general population. Also, if a relatively healthy patient has a parent that has developed type 2 diabetes, there are 3 times more likely to develop type 2 diabetes themselves.

Is there a cure?

Currently, there is no cure for type 1 or type 2 diabetes, but it can be treated by administering the insulin hormone, either by injecting it or pumping it. Besides adding the extra insulin you need to get the glucose into your bloodstream, you also need to follow a healthy and balanced diet as well as making sure you are getting enough exercise every day. When it comes to the schedule of your diabetes medication, a lot of planning and attention is required. This feels overwhelming to some patients sometimes, however, this doesn’t mean that you or someone close to you has to stop living a happy and healthy life. Effective diabetes care can only be achieved through listening and working closely with your healthcare providers. However, the most important person in managing your diabetes is yourself, making the correct decisions is important for continuing a happy and healthy life.

Although the genes we inherit can have an effect on the development of the disease, there are always behavioral and lifestyle factors that you can take advantage of to avoid diabetes. Data from certain health studies conducted by nurses suggest that 90% of type 2 diabetes in women can be attributed to 5 factors: an unhealthy diet, excess weight, smoking, lack of exercise, and an abundance of alcohol consumption. Women who ended up in the low-risk group were 90 percent less likely to have developed diabetes. This low risk group all had common factors including a healthy weight, a healthy diet, at least 30 minutes of exercise on a daily basis, no smoking, and control the amount of alcohol they consumed per week. Almost identical factors were found in the results of male patients.

Syphilis, a chronic bacterial disease, is most commonly developed by infection during sexual intercourse, and in rare cases by the infection of a developing fetus. Even in big cities in the United States, such as New York, doctors and specialists are seeing a large increase in not only syphilis but other sexual transmitted diseases such as gonorrhea and chlamydia.

Doctors and researchers are still trying to work out why sudden significant increases of syphilis are being reported now. Despite a lot of effort and tests, health officials cannot pinpoint the exact causes. Some researchers say it is because technology becoming more common in people’s lives. Recently, a growing number of people are having casual sex through dating apps such as Tinder and Grindr. More casual sex leads to more sexual intercourse between people that don’t know each other sexual past. However, there has been no conclusive evidence that these apps are the reason behind the increase in syphilis cases.

Half of the world’s population has Helicobacter Pylori (H Pylori) infection and 8 out of 10 people have no related symptoms. An ulcer is a sore in the lining in the stomach and its symptoms are abdominal pain, nausea, bad breath, and vomiting.

One way H Pylori is thought to be transmitted is through contaminated water and food. Developing countries have a higher rate of infection, but H Pylori can be treated with antibiotics, such as amoxicillin or tetracycline as well as either a proton pump inhibitor like omeprazole, which suppresses stomach acid.

“Breakbone fever” Dengue Fever

Approximately 390 million cases and anywhere between 10 to 20 thousand deaths occur from the dengue fever per year. The widespread of the dengue virus has intensely developed in the last ten to fifteen years resulting in almost half of the world’s population being at risk. This epidemic originated in tropical areas but has been recently traveling across Europe and America.

Dengue is a disease transmitted by a mosquito bite with symptoms beginning 3 to 14 days after infection. They include high fever, vomiting, headache, muscle, and joint pains, and various skin rashes. A small percentage of people can develop a life-threatening stage called dengue hemorrhagic fever. The symptoms of this would include abdominal pain, frequent vomiting, and low levels of blood platelets; some might go into dengue shock syndrome where their blood pressure becomes dangerously low. The dengue fever has three phases; febrile, critical, and recovery. The diagnosis of dengue fever is based on related symptoms, physical examinations, and various test kits.

This disease has been called “breakbone fever” as it causes severe joint and muscle pain, and also dehydration. Another symptom of “beakbone fever” is hemoconcentration; this means that there is a decrease in plasma volume, consequently an increase in the concentration of red blood cells. This can cause bleeding; usually from the nose or gums. There has been no precise cure for dengue fever; we can only try to manage the side effects that occur; treatment has been pain-relievers (Acetaminophen) and fluids (IV). The use of anti-inflammatory drugs and corticosteroids should be avoided. A vaccine called Dengvaxia has been discovered by Sanofi Pasteur and is waiting for approval by the WHO, but is already being administrated in high-risk areas. “Dengvaxia” is a combination of a diluted version of the yellow fever virus and four serotypes of the dengue disease. It has been proven to be 60% effective and prevents 8 out of 10 severe cases.

Mosquitoes live and breed on still water and feed on the nectar of plants, but females also need to get protein to make eggs; which they get from the blood of animals or humans. One way to get rid of the dengue fever and other mosquito-borne illnesses is to exterminate mosquitoes all together in heavily populated areas and high-risk environments. On a global scale, we can do this naturally by producing parasites that will exterminate mosquitoes such as fungi (Bacillus Thuringiensis and silver nanoparticles). Also by breeding various predators like dragonflies and Gambusia Afins (a type of fish) and nanoparticles. Another approach is to bioengineer sterile males that will prevent the next generation of mosquitoes to be born. For the average person, they can use a repellent with at least a 10 percent concentration of DEET. Furthermore, you can eliminate standing water, wearing long-sleeved shirts and pants to prevent bites, and using screens or nets to avoid mosquitoes from coming in.

Another 1.5 Million lives Worldwide will be taken by tuberculosis this year (2016).
According to a report from the World Health Organization, tuberculosis has surpassed HIV as the leading cause of death from an infectious disease since 2014. As a reference, more than 1000 residents including infants are already infected with TB (tuberculosis) in California on April 5th, 2016. Moreover, Vietnam is running out of funds to fight tuberculosis since the 28th of March, which was important enough for the New York Times to consider this serious incident as the main cover. These two incidents remind us of the war against tuberculosis is still an on-going battle.

What’s going on in Vietnam?
The biggest threat in Vietnam is the absence of financial support.

Twenty-five years ago, due to prolific poverty and a heavy-handed government, Vietnam had nearly 600 cases of tuberculosis for every 100,000 residents and had to suffer desperately over the years. However, because of the great continuous effort attempted by the Vietnamese government, the number of cases has been down to less than 200. Along with these efforts, the Vietnamese government accomplished a 90 percent cure rate for uncomplicated tuberculosis and 75 percent of its more resistant cases, which surprisingly beat the global average of 50 percent.

Vietnam has set another ambitious goal of lowering the prevalence rate to 20 cases per 100,000, which essentially links to tuberculosis (TB) as a public health problem. In order to implement this bold goal Vietnam needs to spend at least 66 million USD a year, but they are unfortunately capable of spending about only 26 million USD for this year, and nearly 19 million USD of the current financial support comes from foreign donors, from this more than one-third of the support comes from the United States. Therefore, if the United States decides either not to fund anymore or to lower the funds in the future, Vietnam will, most likely, face a serious consequence and won’t be able to accomplish their ambitious goal.

Along with their high dependence from financial support from foreign countries, Vietnam’s fascinating accomplishments of preventing tuberculosis are very fragile because hospital wards in Vietnam are packed to their limits. This raises the risks of drug-resistant strains to easily spread out. Therefore, Vietnam is in serious need of financial support to fund their fight against this disease.

The current stage against tuberculosis(TB)
As shown above, tuberculosis has become the most serious virus since 2014 and the impact it has surpassed the impact of HIV. Worldwide, the mortality caused by both HIV and tuberculosis has been dropping year by year, but the number of deaths by tuberculosis has decreased more slowly than HIV’s, especially in Asia. According to a report by the World Health Organization, there were close to 10 million new cases of the diseases and 1.5 million deaths in 2015. Additionally, more than 4 thousand people are dying every day from tuberculosis.

The 20th Annual Tuberculosis Report shows that the incidence of tuberculosis has been reduced annually since the 1990s with great effort from the global health organizations and all countries across the globe. However, the threat remains.

WHO is calling on countries and partners to “unite to end tuberculosis”
Millennium Development Goals planned by the United Nation had a great impact worldwide in reducing the number of incidences. Furthermore, from 2000 to 2014 almost 43 million lives were saved because of better diagnosis and treatment. A US initiative, the Sustainable Development Goals (SDGs) is to end tuberculosis by 2030 as an intergovernmental set of aspiration goals with 169 targets. The specific goal is to reduce tuberculosis deaths by 90 percent and tuberculosis cases by 80 percent by 2030. To achieve this ambitious goal, better tracking systems are required to be available at any time and any place on a global scale. Even though enough funds come from many nations to implement the program, tuberculosis has become resistant to the existing drugs used to treat it as a future cure.

We all live in a world where boundaries between nations are shrinking and becoming more interrelated with one and other as a financial crisis from one country can impact the whole world. Therefore, we need to keep our eyes on the situation in Vietnam and should not treat this incident as a separate concern of a foreign country. Who knows how tuberculosis would impact our world once again like what happened before in the 1900s.

Hawaii outbreak

On February 9, Hawaii has declared a state of emergency on their island as it controls the local spread of dengue fever. Experts think it may foreshadow how Zika Virus transmission might start in parts of the United States.
The declaration of emergency potentially could help Hawaiian authorities get funds to control mosquitoes that may transmit the virus, which in turn could support the state lower the risk of the local spread of Zika.

As a precaution, officials have decided closing down certain areas in Hawaii that were considered major spots in the dengue fever outbreak.

227 Hawaii residents and 24 visitors have been affected by dengue so far. Out of this 206 are adults and 45 are children. However, as a piece of good news, there is no significant sign of infections after the declaration of emergency has shown.

Cases in Brazil

On February 12, a nearly 50 percent jump in cases of dengue fever reported over a three-week period in January, Brazil’s government reported. 74,000 “potential” cases of dengue from January 3 to January 23, which is an increase of 50 percent from a similar period in January 2015, Brazil’s Health Ministry reported.
Brazil has been suffering by the spread of Zika Virus and the increasing number of babies born with unusually small heads as the Zika virus is linked with newborn microcephaly.

Since dengue fever, Zika, and other diseases such as Chikungunya are spread by the same mosquitos – the Aedes Aegypti species, the Brazilian government is seriously attempting to slow down the increase in numbers of these infectious mosquitoes, which lay their eggs in stagnant waters.

Marcos Lago, an associate professor of infectious diseases and pediatrics at the State University of Rio de Janeiro, said: “We will probably have a dengue epidemic”, while adding: “And this dengue epidemic will be accompanied by a Zika epidemic.”

Symptoms of Dengue Fever

People, especially teens and children, may not experience signs or symptoms during a mild case of dengue fever. After people are bitten by an infected mosquito, four to ten days later symptoms start to occur.

The common symptoms of dengue fever are listed below:

  • Fever as high as 106° F (41° C)
  • Headaches
  • Muscle, bone and joint pain
  • Pain behind your eyes

Some might also experience the following symptoms:

  • Widespread rash
  • Nausea and vomiting
  • Rarely, minor bleeding from your gums or nose

The majority of people usually recover within a week or so. However, in many cases, symptoms worsen and can become life-threatening. Blood vessel often gets damaged and leaky. And the number of clot-forming cells in bloodstream drops. This can possibly cause:

  • Bleeding from your nose and mouth
  • Severe abdominal pain

Treatment

Hepatitis refers to an inflammation of the liver caused by the presence of inflammatory cells in the liver. These cells are responsible for almost all human forms of hepatitis. Patients infected can have few or no symptoms, but common hepatitis symptoms include nausea, vomiting, fever, weakness, loss of appetite and aching in the abdomen. The confirmation of hepatitis infection is made through a specific blood test for each type. Hepatitis differs between acute and chronic illnesses, depending on the type and time length of the ongoing hepatitis infection. During the period of infection, the disease is acute, but it is vulnerable to become chronic after six months of infection. According to researchers, the most common hepatitis viruses are types A, B, and C.

HEPATITIS A AND E

Hepatitis A (HAV) and Hepatitis E (HEV) are spread by contaminated food and water especially where unsanitary conditions allow them to be in contact with human waste (fecal-oral mode of transmission). It is spread through close contact and the passage of oral secretions, such as intimate kissing, or poor hand washing. Hepatitis E mainly occurs in Asia. The biggest difference between HAV and HEV is that in pregnant women HEV can cause severe liver failure and elevated mortality rate. There is an effective HAV vaccine for prevention, but there is no official vaccine for HEV. Some vaccines produced in Southern Asia have been shown to be effective, but have not been made available to the general populace. The development of the vaccine has been stopped in developed countries for economic reasons since HEV is rare.

HEPATITIS B AND D

Hepatitis B (HBV) and Hepatitis D (HDV) are spread through infected blood, bodily fluids, and contaminated needles. They can be transmitted by shared needles in drug abusers, tattooing and body piercing, from infected mothers to their newborns, by sharing toothbrushes contaminated with infected blood, or through blood transfusions, and hemodialysis. To be infected by Hepatitis D, the patients must already have hepatitis B. HDV cannot survive on its own because it requires a protein that HBV produces to enable it to infect liver cells. About 6 to 10% of patients with HBV hepatitis develop chronic HBV infection, which means they can infect others as long as they remain infected. They have a high risk of developing cirrhosis, liver failure, and liver cancer. There is an effective HBV vaccine for prevention, which also prevents HDV.

HEPATITIS C

Hepatitis C (HCV) usually is spread by shared needles among drug abusers, blood transfusion, hemodialysis, and needle sticks. According to researches, 90% of transfusion-associated hepatitis is caused by HCV. It can also be transmitted by sexual contact but it is considered rare since it requires blood-to-blood contact. The highest risk group is drug users who share needles. About 75 to 85% of patients with acute HCV develop chronic HCV infection. Moreover, they have a high risk of developing cirrhosis, liver failure, and liver cancer. There is no Hepatitis C vaccine for prevention.

The treatments for Hepatitis differ according to the type and whether is an acute or chronic illness. For acute hepatitis, the initial treatment consists of relieving the symptoms of nausea, vomiting, and abdominal aching. Treatment of chronic hepatitis involves combinations of medications to eradicate the virus. For hepatitis C infection, medications include injectable interferon and oral pills such as ribavirin and boceprevir. Hepatitis B infection treatment includes injectable alpha interferon and oral pills such as lamivudine, adefovir, and entecavir. During the period of treatment for any hepatitis type, the patient must abstain from drinking alcohol and smoking cigarettes since they are toxic to the liver and drink the liquid in large quantities to avoid dehydration caused by vomiting.
